What is contained within the library?
The library contains downloadable versions of all of the FMCSA forms a medical examiner might use when performing an exam. This includes the exam form and certificate, both the Insulin-Treated and Non-Insulin-Treated Diabetes Mellitus Assessment forms, the Vision Evaluation Report, and the CMV Driver Medication Form. Each of the exemption and SPE applications are available to view and download as are the FMCSA handbook and various FMCSA Medical Review Board and Medical Expert Panel Recommendations.
