Features a laminated "original" copy for the driver, and a non-laminated copy for the medical examiner or motor carrier. This bi-fold design (folded once each way) becomes the size of a credit card for easy keeping in a wallet. Instructions regarding the "easy to perform" self-lamination procedure are provided on a tear-away tab.
