Laminated Medical Examiner's Certificate (Tri-Fold Version). See Below regarding expiration dates & FMCSA Compliance!
FMCSA has changed the certificate expiration date from 12/31/2024 to 03/31/2025. However, their bulletin states:
"Please note that the only change to the forms is the change in the OMB expiration date from 12/31/2024 to 03/31/2025...... However, MEs that have purchased these forms in bulk, or require time to reprogram electronic system forms, may continue to use the old forms until stocks have been depleted or forms have been reprogrammed..... Based on this guidance, SDLAs are encouraged to continue to accept these forms."
TeamCME will continue to use and sell our 12/31/2024 certificates until supplies run out.
This Tri-fold version of the self-laminating MEC uses the same height to width proportions as MECs that are being sold by other suppliers. Because this version does not strictly meet the FMCSA and OMB height and width proportions and stretches the printed formatting, we refer to this version as "Not Fully FMCSA Compliant". However, this form has never been rejected.
It features a laminated "original" copy for the driver, and a non-laminated copy for the medical examiner or motor carrier. This tri-fold design is for easy keeping in a wallet (slightly larger than a credit card). Instructions regarding the "easy to perform" self-lamination procedure are provided on a tear-away tab.
