Unemployed Driver Return to Duty Reporting Request
This form must be completed by the driver. Please copy the link below and provide it to them.
Steps to Take:
Step 1: Register in the Clearinghouse as an owner-operator. Download the step-by-step instructions.
- Create a new Login.gov account
- Create a new account in the Clearinghouse with the “Employer” role
- Select “Register as an Employer and Driver”
- Select “Yes” for the question “Are you an owner-operator?”
Note: Drivers should not apply for a U.S. DOT Number when using this option to complete the RTD process.
Step 2: Designate TeamCME as your consortium/third-party administrator (C/TPA) for the limited purpose of completing the RTD process
Step 3: Obtain a negative RTD test
Step 4: Complete the form below
TeamCME will then report the negative RTD test on your behalf.
Your Clearinghouse status will change from “prohibited” to “not prohibited”.
You are then allowed to resume driving a CMV and performing other safety-sensitive functions.
