TeamCME founder Dr. Michael Megehee has been training medical examiners to perform DOT physicals since 1996.

He was selected by the Federal Motor Carrier Safety Administration (FMCSA) as one of the subject matter experts who helped design the original NRCME training curriculum and write the first certification exam questions. His contribution helped establish the national standard for examiner certification.

In 2012, FMCSA Administrator Anne Ferro recognized his contributions by naming him an NRCME Champion. Dr. Megehee has also contributed to updates of the NRCME exam and published in national compliance and healthcare journals.

Dr. Jeff Carlson, TeamCMEโ€™s Chief Medical Officer, co-developed this course and leads the development of TeamCMEโ€™s training programs. He has published on DOT medical compliance and examiner best practices, and has trained examiners nationwide through live NRCME courses.

Together, Dr. Megehee and Dr. Carlson have trained thousands of certified medical examiners nationwide.

How does NRCME recertification work for DOT medical examiners?

The NRCME recertification process closely mirrors initial certificationโ€”medical examiners complete online recertification training and pass the NRCME exam before their certification expires.

Every 10 years, certified examiners must complete training and testing to remain listed in the FMCSA National Registry. After finishing your course, upload the completion certificate to your NRCME account, schedule the exam with Prometric or PSI, and pass prior to expiration to maintain your ability to perform DOT physicals.

When can I start the NRCME recertification process?

You can begin recertification once you reach the nine-year mark of your current certification.

FMCSA rules allow examiners to start training and testing after nine years, giving a full year to complete both steps. Starting early helps prevent lapses if you need to retake the test or experience scheduling delays.

Will I lose the time left on my current certification if I recertify early?

No. Your new ten-year certification period starts from your original expiration date, not the date you complete recertification.

For example, if your certification expires in June 2027 and you complete your recertification in March 2027, your new certification will be valid through June 2037, not March 2037. Renewing early doesnโ€™t shorten your certification- it simply ensures continuous coverage without a lapse.

How can I check the expiration date of my NRCME certification?

Log in to your FMCSA National Registry account, click My Profile then Manage User Informationย and view the โ€œCertification Expirationโ€ field.

The expiration date appears in the upper-right section of your profile. You must complete the recertification process before that date to remain active on the National Registry.

What happens if I fail the NRCME recertification test? Is there a pass guarantee?

You no longer have to wait 30 days to retake the NRCME exam- you can retest as soon as the next day if thereโ€™s an appointment available.

If your certification is still active, you remain listed in the National Registry and may continue performing DOT physicals while you retake the test.

Can I keep performing DOT physicals if I fail the NRCME recertification test?

Yes. As long as your certification has not expired, you can continue performing DOT physicals even if you donโ€™t pass the recertification exam on your first attempt.

Failing the test does not remove you from the FMCSA National Registry or suspend your certification status. Youโ€™ll stay listed as an active medical examiner until your current certification expires, giving you time to retake and pass the exam without any interruption to your practice.
