You can complete our intake forms and driver history before your appointment or in our waiting room, which only takes a few minutes.

About the office

Our office is conveniently located along I-84 with a dedicated exit (Exit 71) and offers free parking for over 150 trucks. Waypoint Wellness has partnered with the historic Boise Stage Stop to provide the most stress-free DOT physical experience in the Pacific Northwest. Our clinic is situated on the second floor, right next to the drivers' lounge and gym.
