About the provider

Dr. Richard Kates

Dr. Richard Kates graduated from Cleveland Chiropractic College in Los Angeles in 1997. After practicing in California for two years, he moved to Kentucky to be near his family. In 2001, he opened his practice in Carrollton, Kentucky. Some of the services offered at his practice include DOT and non-DOT drug testing and alcohol screening. He is a DOT Certified Medical Examiner who performs both DOT and non-DOT physical exams. Currently, Dr. Kates resides in Carrollton with his wife and four children.
