Important: Before submitting this form, you will be required to provide ONE of the following:
Option 1: Provide a DOT drug test result for each participant from within the last 30 days.
Option 2: Provide proof of previous enrollment in a randomized drug and alcohol testing program within the last 30 days, consisting of either:

- A certificate of enrollment OR
- Have the company complete our Prior Consortium Enrollment Verification Form. Once completed, both you and the company will receive a PDF that must be uploaded in the designated field below.
